Bereavement Services

Outreach provides support, education and facilitation of community resources to meet the special needs of patients with serious and chronic illness, and the needs of their families. This program is supported through community and private donations.

“I am glad I got the nerve to come to grief recovery. It really helped me through a very rocky time.
– HM Patient Family Member

Day and night respite offers relief for the primary caregiver when a caregiver cannot assume full-time care of the patient during the day or perhaps just needs a full night of uninterrupted rest. Work respite is also an option for the caregiver who must work 40 hours a week and has no back up.

Crisis intervention is provided to assist a family with an overwhelming physical or emotional condition that arises. Skilled caregivers are placed in the home around-the-clock to help manage the symptoms of the crisis and assist the family in keeping the patient at home, where he or she has chosen to be.

Bereavement and Support Groups
All groups are open to the community. There is no charge.
